Not long ago, I met with one of my PEA-SimpliMD coaching clients, Dr. Angela Martinez (name changed for privacy). She arrived at our Zoom call with tired eyes, shoulders slumped under the invisible weight of her full-time W-2 job.
"I feel like I’m being crushed," she admitted. "I want to get out. I want to practice on my own terms. But the thought of going fully independent overnight is terrifying. I don’t even know where to begin."
Her story was all too familiar. The combination of bureaucratic overload, loss of autonomy, and misalignment with her values had drained her. Yet, she wasn’t asking for a grand exit strategy. She was asking for permission to go slow. To thoughtfully unfold her entrepreneurial path without detonating her career or her finances.
That’s when we discussed the idea of downshifting.
Start With Downshifting Your W-2 Job
I told her something that many physicians don’t realize: You don’t have to choose between all-in employment and full-time entrepreneurship. There is a smart, stable middle path. By reducing your traditional W-2 employment to 0.6 to 0.8 FTE, you create time and mental space to experiment, explore, and build.
Together, we mapped out how she might approach her employer to request a reduction in hours—highlighting how she could still meet quality metrics, maintain patient continuity, and be an asset to the team. Our goal wasn’t to burn bridges; it was to build margin.
Reduce Burnout Risk
Angela was clearly burning out. Like so many physicians, she had fallen into the trap of endless inbox messages, unnecessary meetings, and performance metrics that didn’t reflect her clinical excellence. By reducing her clinical hours, she could reconnect with her passion and regain control of her professional life.
Test Independence Gradually
With her new schedule, Angela didn’t need to dive into full-time business ownership. She could test 1099 telehealth roles, explore cash-based chronic care services, and begin consulting without jeopardizing her stability. This removed the pressure and allowed her to iterate her next chapter.

Maintain Benefits
We verified that her employer offered prorated benefits for physicians at or above 0.5 FTE—a huge win. This meant she could maintain health insurance, retirement contributions, and malpractice coverage while carving out her new direction.

I encouraged her to use this time to begin developing her professional brand outside the walls of her employer:

This hybrid model—part W-2, part micro-business owner—gave her the flexibility to design a career aligned with her values. It wasn’t just about survival; it was about sustainability and sovereignty.
