I see the same pattern repeatedly in coaching calls. A physician forms an LLC online in ten minutes, opens a bank account, and starts contracting. Six months later they realize their tax setup is wrong, payroll is misclassified, benefits were missed, and asset protection is thin.
That is not entrepreneurship. That is improvisation.
A micro business is not complicated, but it is sequential. Order matters. When you skip steps, problems surface later, and usually when the stakes are higher.

Step 3: Choose the correct entity PLLC or PC first. Tax classification second. Confusing these two decisions are one of the most common physician mistakes.
Step 4: Legal and regulatory setup EIN, licensing alignment, HIPAA considerations, and compliance infrastructure should exist before revenue flows.
Step 5: Financial separation Business banking, bookkeeping, and payroll must be clean from day one. This protects you legally and financially.
Step 6: Insurance and liability protection Malpractice, business liability, and asset protection strategies belong here, not later.
Step 7: Operational design Virtual or physical. Solo or supported. Contractors or employees. You design intentionally instead of inheriting defaults.
Step 8: Market positioning Your micro business must communicate who it serves and why it exists. Clarity attracts the right work.
Step 9: Household benefits strategy Health insurance, retirement plans, fringe benefits, and tax planning are part of the business, not separate from it.
